The day-by-day timeline

Day
0

Procedure day

You return to the hotel after your appointment. Rest for the remainder of the day. Mild swelling begins within the first hour and peaks at 24–48 hours.

  • Apply cold compresses 15 minutes on, 15 off, for the first 4 hours (wrap ice pack in a towel — never apply ice directly to skin).
  • Take oral paracetamol (acetaminophen) 500–1000 mg as needed for discomfort. Avoid ibuprofen and aspirin for 72 hours (they thin the blood).
  • Drink water normally. Eat normally.
  • Do not shower for the first 6 hours. After that, warm (not hot) showers are fine. No long baths, no hot tubs.
  • Wear loose boxers. Avoid tight underwear, tight trousers, tight shorts.
  • No sexual activity of any kind. No self-stimulation. No erections if you can avoid them.
  • Sleep on your back if possible. No pressure on the treated area.
Day
1

First full day post-procedure

Expect peak swelling. The tissue will feel firm. Mild bruising at the cannula entry point is possible. You can walk, move around, and leave the hotel for light activity (meals, short walks).

  • Continue cold compresses 4–6 times per day.
  • Continue loose clothing and rest.
  • Drink 2+ litres of water (helps the filler settle and distribute).
  • Avoid alcohol (delays healing, increases bruising).
  • No gym. No running. No heavy lifting.
  • No sauna, steam room, Turkish hammam, or hot bath.
  • Send us a photo via WhatsApp if you would like a check-in.
Day
2–3

Stable phase, swelling beginning to reduce

Swelling begins to reduce noticeably. Any bruising is at its most visible and starts to fade. Most international patients fly home on day 2 or day 3.

  • Flying is not contraindicated — the filler is not affected by altitude or cabin pressure.
  • Wear loose travel clothing. Stand up and walk every hour during long flights.
  • Resume normal eating and drinking (including modest alcohol, if you wish — but avoid heavy drinking).
  • Light desk work is fine. Physical labour or heavy activity is not.
  • Still no sexual activity.
  • Still no gym, sauna, or hot bath.
Day
4–6

Return to most daily activity

Swelling continues to reduce. The tissue is settling. Any bruising fades. You should be feeling essentially back to normal on day 5–6.

  • Normal work and activity can resume.
  • Light exercise (walking, gentle stretching) is fine.
  • Still avoid heavy gym work, running, and cycling (saddle pressure).
  • Still no sexual activity.
  • Can resume warmer showers. Still no sauna or very hot baths.
  • Standard underwear is fine from day 5.

7–10

Gentle resumption of sexual activity

Sexual activity — both solo and partnered — can gently resume from day 7 for most patients. Be cautious on the first session: no vigorous friction, no aggressive manipulation, no unusual positions. Think "gentle reintroduction" rather than "back to normal".

  • Normal gym activity can resume from day 8–10 (cycling can wait until day 14).
  • Saunas, steam rooms, hot baths: can resume from day 10.
  • Swimming in chlorinated pools: day 7 onwards. Sea/lake/natural water: day 14 onwards.
  • Avoid aggressive penile pumping, jelqing, or traction devices for 6 weeks (they can displace filler before it has fully integrated).

10–14

Integration phase

Filler is now integrating with the surrounding tissue. Residual swelling continues to resolve. The tissue feels softer and more natural with each passing day.

  • All normal activity is fine.
  • Sexual activity with normal vigour is fine.
  • The final settled result is approaching — you are seeing ~90% of what you will see at the full 4-week mark.

14–28

Final settling and review

All swelling has resolved. Filler is fully integrated. The tissue feels natural to touch for both patient and partner. This is the endpoint that we measure, photograph, and compare to baseline.

  • 4-week remote review: either a clinic visit if you are in Istanbul, or a video consultation and photo upload if you are home.
  • Any minor asymmetry is reviewed; if a correction is clinically indicated, it is arranged at this point (typically no additional charge if within 30 days).
  • Final measurements recorded in your patient file.
  • 30-day WhatsApp aftercare support period concludes. The clinic remains available for questions at any point beyond this.

months

Mid-term check-in

Optional voluntary check-in at 6 months. We ask a short satisfaction questionnaire and offer any advice needed. No cost.

12–14
months

Top-up window

Many patients begin to notice a gradual reduction in circumference at 12–14 months as the HA is progressively metabolised. This is the optimal window for a top-up — typically half the original volume is sufficient to restore the original result. See Pricing for returning-patient rates.

Symptom FAQ during recovery

"The tissue feels firm. Is this normal?"

Yes — expected. Filler is denser than native subcutaneous tissue during the first 1–2 weeks, and there is residual swelling. By day 14, softness is approaching normal; by day 28, it is indistinguishable from native tissue to touch.

"I see a small lump. Should I be worried?"

Not necessarily. Small, firm areas that resolve over the first 2–4 weeks are normal settling behaviour. A persistent lump that does not resolve — or a lump that grows, becomes tender, or changes colour — should be reported. We will review by photo or video call and decide whether a short course of localised hyaluronidase is indicated.

"There is mild bruising at the entry point."

Normal. The cannula enters through a single point on the ventral (underside) aspect of the shaft. Mild pink or light purple discolouration at this point is expected for 3–7 days. Full resolution within 10 days for most patients.

"I had an unexpected erection on day 2. Did I damage anything?"

Almost certainly not. The filler is in the subcutaneous tissue plane, outside the erectile bodies; an erection does not disturb it at this stage. If you are concerned, photograph the area and send the image through WhatsApp — we will review within the hour.

"My partner and I want to be intimate. When can we?"

Gentle resumption from day 7. Full resumption from day 14. The first session after the procedure is not the time for novelty — we recommend a relaxed, familiar setting.

"I have a flight on day 2. Is that safe?"

Yes. The filler is not affected by cabin pressure. Wear loose clothing, stand up and walk periodically on long flights, and stay well hydrated. Do not, however, plan an ambitious post-flight evening — you should be resting when you land.

When to contact us immediately

Most recovery is uneventful. That said, use the 30-day WhatsApp line (or, if we have responded to an earlier message, reply to that thread) if any of the following occurs:

  • Pain is increasing rather than decreasing after day 2.
  • New redness, heat, or swelling after day 3 (original swelling should be subsiding, not restarting).
  • Any discharge from the entry point after day 2.
  • Fever, feeling generally unwell, or signs of systemic infection.
  • Skin colour change — dusky, blue, or white patches.
  • Sudden loss of sensation — not mild reduction, which is expected, but sudden loss.
  • Any new difficulty passing urine (exceptionally unlikely, but to be reported).
  • A persistent, enlarging lump that does not settle over 2 weeks.
  • Anything — anything — that is worrying you. We would always rather have the message.

Our 30-day WhatsApp aftercare channel

The channel is activated on your phone before you leave the clinic. You have direct, 2-hour-response access to our clinical team (and to Prof. Dr. Sertkaya directly for any clinical concern that requires physician input). The channel stays open for 30 days as standard and can be extended by request — we do not charge for extending it.

Long-term maintenance

After the first month, there is essentially nothing specific to do. The filler gradually metabolises over 12–24 months, and the tissue reverts slowly toward baseline. Many patients schedule a top-up at 12–14 months to maintain the result. Some choose to let the filler fully resorb and then re-evaluate.

There are no long-term behavioural restrictions. Use of the penile vacuum pump, traction devices, or jelqing is not contraindicated after the filler has fully integrated (typically 6 weeks), but we recommend moderation — aggressive mechanical stress over months can accelerate filler migration in some patients.
